This record provides the standardized English master questionnaire used to survey Special Olympics athletes and coaches in Ukraine, Germany, and the Czech Republic (March–May 2025). The instrument operationalizes Program Access and Quality (PAQ-SO), Perceived Organizational Support (POS-SO) – with sport-specific and psychosocial/humanitarian subscales – and psychosocial outcomes (PSS-4 stress, ULS-3 loneliness, BRS resilience). It includes response scales, scoring rules, accessibility guidance, and an assistance protocol to ensure neutral support for participants with intellectual disabilities. Certified translations (Ukrainian, German, Czech) were derived from this master via forward- and back-translation.
